General Information

Please bring all insurance cards and insurance information with you to your appointment.

Many medical insurance companies and HMOs require referrals or pre-authorizations for certain medical services. If your plan requires authorization, please contact your insurance company or primary care physician before your appointment.

We accept cash, checks, major credit cards, and CareCredit.

Co-payments, deductibles, refractions, and any non-covered services are due at the time of service.
