Insurance Disclaimer

As a courtesy, we verify insurance eligibility and benefits before your appointment whenever possible. We make every effort to help you understand and maximize your benefits.

However, insurance information provided to our office is not a guarantee of payment. Benefits may change due to:

● Policy changes

● New benefit years

● Changes in insurance coverage

● Claims processed after eligibility verification

● Purchases made after benefits were verified

Any charges not covered by your insurance plan, including co-payments, deductibles, coinsurance, and non-covered services, remain the patient's responsibility.

Vision Plan Information

Many vision plans have specific rules and limitations that we are required to follow. Our team will do our best to explain your benefits and any applicable restrictions before services are provided.

Insurance plans and policies change frequently. While we strive to remain current and accurate, adjustments to eyewear orders may occasionally be necessary. If changes are needed, we will contact you before proceeding.

If you discover vision insurance coverage after services have already been provided, we are unable to retroactively change coding or rebill completed transactions due to insurance regulations. However, we will gladly provide an itemized receipt that you may submit directly to your insurance carrier for possible reimbursement.
